Private Lesson Contract 2024

This Service Agreement is entered into between Treble Makers Music LLC, represented by its owner and educator, Candace Blair, and the undersigned.

Location of Lessons

Private music lessons will be conducted at the Treble Makers Music studio located at:

2728 Colonial Ave. SW Suite 106,
Roanoke, VA 24015.

Lesson Fees

Each private lesson is priced at $25 for a duration of 30 minutes.

Payments for the entire month are to be made at the beginning of each month. Accepted payment methods include cash (Exact Change), check, or major credit cards.

Cancellation Policy

Any missed lessons will be charged unless a 24-hour prior notice is provided to Educator Candace Blair. If the notice is provided timely, the lesson fee will be adjusted for the subsequent month's lessons. No refunds are provided. If lessons are canceled by the Educator due to illness or adverse weather, the payment will be accounted for in the next month's lessons.

Lesson Requirements

Students must bring all required materials to each lesson. Regular practice at home, based on the instruction given during lessons, is crucial for progression.

Health and Property

Please check all:

The undersigned agrees not to send their child to lessons if they are unwell.

Neither Candace Blair nor Treble Makers Music is liable for lost personal items.

The Educator reserves the right to discontinue services at any time.

The undersigned acknowledges understanding and agreement to the terms stipulated above.

Media Consent:

Grant permission for Treble Makers Music and Candace Blair to photograph or record the student during lessons for educational or promotional purposes.

Amendments

Any changes to this Agreement must be in writing and signed by both parties.

Governing Law

This Agreement shall be governed by the laws of the state of Virginia.
