Winter/ Spring 2024 Ukulele Contract
This Contract of Services is made effective Thursday January 11th, 2024 and terminates Thursday May 9th, 2023. (Advanced students: Monday January 8th- Monday May 6th)
The educator and owner of Treble Makers Music, Candace Blair, will provide 17 group Ukulele classes out of her studio located at 2728 Colonial Ave Roanoke, Va. 24015 office suite 106.
Payment is due in full before the class begins to ensure your spot in the class in the amount of $425.00. Cash in exact change, Check, and all major credit cards are accepted in the studio. A Payment Plan can be arranged upon request.
If you are a New Beginning student a one time activity fee of $60 is due with the semester payment which includes a Ukulele and all materials needed for the class. Returning students do not pay this fee.
No refunds will be presented. In the event of a canceled class due to inclement weather or teacher illness the teacher will provide a makeup class at the end of the semester. No Makeups will be provided individually.
Beginners meet Thursdays from 3:00-4:00
Intermediate Students meet Thursdays from 4:00-5:00
Advanced Students meet Mondays from 4:00-5:00
The teacher Candace Blair has the right to terminate services at any time. I agree to all terms listed in this contract.
MEDIA CONSENT: By selecting one of the options below, the client provides or denies permission to Treble Makers Music to photograph or record the child during classes or performances for educational and promotional purposes.
Grant permission for Treble Makers Music and Candace Blair to photograph or record the student during lessons for educational or promotional purposes.
Amendments
Any changes to this Agreement must be in writing and signed by both parties.
Governing Law
This Agreement shall be governed by the laws of the state of Virginia.
